The Widal tube method is a serum agglutination test to diagnose typhoid fever by detecting antibodies against Salmonella Typhi. A blood sample is taken and mixed with different dilutions of typhoid antigens in tubes. After incubation, the highest dilution showing visible clumping (agglutination) is the patient's antibody titer. A positive test indicates infection, but a single result is not definitive; a significant rise in titer over several days is suggestive of an active infection. Purpose Diagnose Typhoid Fever: The test helps confirm a typhoid fever diagnosis caused by Salmonella typhi or paratyphi. Antibody Detection: It measures the level of antibodies in the blood that the body produces in response to the Salmonella bacteria. Procedure Blood Sample Collection: A trained phlebotomist draws a small blood sample from a vein. Serum Separation: The blood is allowed to clot, then centrifuged to separate the serum from the blood cells. Serial Dilution: The collected serum is then diluted in a series of tubes to create different concentrations, typically from 1:20 up to 1:1280. Antigen Addition: A specific concentration of Salmonella antigen (O and H antigens) is added to each tube containing the diluted serum. Incubation: The tubes are incubated, usually at 37°C, for 16 to 20 hours. Observation: After incubation, the tubes are examined for agglutination, which is the visible clumping of the antigen and antibody complex. Results & Interpretation Agglutination Titre: The "titre" is the highest dilution of serum that shows a clear agglutination. Significant Titre: A significant titre (e.g., 1:80 or higher) is suggestive of infection. Rising Titre: A notable increase in titre (a four- to five-fold rise) in a repeat test after a few days is more strongly indicative of an active infection than a single high titre. Quality Control: Saline controls are used to ensure the test is valid, and a valid saline control should not show agglutination. Important Considerations False Positives: Individuals vaccinated with the TAB vaccine or those who have had typhoid in the past might show a moderate titre, which is known as an anamestic response. Confirmation: Because of potential limitations, a positive Widal test result should often be confirmed with a blood culture for a definitive diagnosis.
