Coombs Test- Principle, Types, Procedure and Result ...The indirect Coombs test detects the presence of antibodies in a person's blood serum that could attack red blood cells. It is used to screen for antibodies before a blood transfusion to ensure compatibility and is a key part of prenatal care to check for Rh incompatibility between a pregnant person and their unborn baby. A positive result indicates that antibodies are present, which can lead to a reaction with donor blood or cause complications for a baby. How it works Step 1: Combine samples. A patient's blood serum is mixed with reagent red blood cells (cells with known antigens). Step 2: Incubate. The mixture is incubated to allow any antibodies in the serum to bind to the red blood cells. Step 3: Wash cells. The red blood cells are washed to remove any unbound antibodies. Step 4: Add Coombs serum. Anti-human globulin (Coombs serum) is added, which is an antibody that binds to human antibodies. Step 5: Observe for clumping. If there are antibodies in the patient's serum that have attached to the reagent red blood cells, the added Coombs serum will cause the cells to clump together (agglutinate). Why it's performed Before blood transfusions: It ensures the donated blood is compatible with the recipient's blood, preventing a hemolytic transfusion reaction. During pregnancy: It screens for Rh sensitization in an Rh-negative pregnant person who is carrying an Rh-positive baby, a condition known as hemolytic disease of the newborn (HDN). To diagnose conditions: It helps in diagnosing autoimmune hemolytic anemia, where the body's immune system attacks its own red blood cells. What the results mean Negative: A negative result is good news and typically means there are no clinically significant antibodies present. For a transfusion, this means the blood is safe to use. In a pregnant person, it means there is no risk of the mother's antibodies attacking the baby's red blood cells. Positive: A positive result means that antibodies are present in the serum. This indicates that the blood is not compatible for transfusion and that the pregnant person's immune system could potentially attack the baby's red blood cells.
