Hepatitis E is a liver inflammation caused by the Hepatitis E Virus (HEV), an enterically transmitted RNA virus. It is primarily spread through the fecal-oral route, mainly via contaminated drinking water, especially in areas with poor sanitation, but can also be transmitted through undercooked meat or by blood transfusions. For most people, the infection is self-limiting and resolves in 1 to 6 weeks with supportive care, but it can lead to severe illness like fulminant hepatitis (acute liver failure), particularly in pregnant women, or chronic hepatitis in immunocompromised individuals. Characteristics of the virus and infection Virus type: Hepatitis E is caused by the Hepatitis E Virus (HEV). Structure: HEV is a non-enveloped, icosahedral particle with a single-stranded RNA genome. Infection: The infection is typically acute and self-limiting in immunocompetent individuals. Severity: While most cases resolve, it can occasionally progress to severe disease, including acute liver failure, especially in pregnant women. Chronic infection: Chronic hepatitis E is rare but can occur in immunocompromised patients, such as organ transplant recipients. Transmission Fecal-oral route: The most common transmission is through the fecal-oral route, often by consuming contaminated drinking water. Zoonotic transmission: Some genotypes (like HEV-3) can be transmitted by eating uncooked or undercooked meat, especially from pigs or wild boar. Other routes: Transfusion-related and vertical transmission (from mother to fetus) have also been reported. Treatment: There is no specific cure for acute hepatitis E; treatment is supportive and focuses on allowing the liver to recover. Severe cases: Patients with severe symptoms like persistent vomiting or signs of liver failure may require hospitalization. For chronic cases in transplant recipients, antiviral drugs like ribavirin may be used.
