A lupus anticoagulant (LA) test is a blood test that detects autoantibodies associated with antiphospholipid syndrome (APS), which increase the risk of abnormal blood clotting and potential complications like deep vein thrombosis or recurrent miscarriages. The test involves screening methods such as the activated partial thromboplastin time (aPTT) or dilute Russell's viper venom time (dRVVT), followed by a mixing study to confirm the presence of an inhibitor and rule out a coagulation factor deficiency. A positive result indicates the presence of lupus anticoagulant antibodies, which requires further investigation and management by a healthcare professional.
Purpose of the Lupus Anticoagulant Test
This test is used to:
Diagnose antiphospholipid syndrome (APS), a condition characterized by recurrent blood clots.
Investigate unexplained blood clots in veins and arteries.
Determine the cause of recurrent miscarriages or other pregnancy complications.
Evaluate a prolonged partial thromboplastin time (aPTT), which can be a sign of a clotting disorder.
How the Test Works
The lupus anticoagulant test is not a single test but a series of laboratory assessments:
1. Screening Tests: These tests measure the time it takes for blood plasma to clot. Common screening tests include:
Activated Partial Thromboplastin Time (aPTT) or LA-sensitive PTT (PTT-LA).
Dilute Russell's Viper Venom Time (dRVVT).
If the screening test shows a prolonged clotting time, a mixing study is performed.
2. Mixing Study: This step involves mixing the patient's plasma with normal pooled plasma to see if the prolonged clotting time is corrected.
If the clotting time remains prolonged, it indicates the presence of an inhibitor, such as a lupus anticoagulant.
If the clotting time corrects, it suggests the prolonged clotting is due to a deficiency in a coagulation factor rather than an inhibitor.
Important Considerations
Not a Diagnosis for Lupus: The term "lupus" in "lupus anticoagulant" is misleading; a positive test does not mean someone has lupus, though some people with lupus do have these antibodies.
Confirmation Test: A positive result from the screening test needs to be confirmed with a repeat test in a few weeks.
Other Tests: Depending on the results, a doctor may order other related tests, such as anticardiolipin antibodies or antiprothrombin antibodies.
