An AFB (acid-fast bacilli) culture of bone marrow is a diagnostic test to detect infections caused by Mycobacterium tuberculosis or other nontuberculous mycobacteria within the bone marrow. It is a crucial test for diagnosing conditions like disseminated tuberculosis, especially when symptoms are vague or other tests are negative, and it can help identify the specific bacteria to guide treatment. How it works Specimen collection: A small amount of bone marrow is collected through a biopsy or aspiration and placed in a special culture vial. Incubation: The specimen is incubated in a laboratory setting for several weeks (often 6–8 weeks) to allow any bacteria to grow. Identification: If bacteria are present, they will be identified through microscopic examination or other methods. This can include using rapid culture systems like BACTEC MGIT. Susceptibility testing: Once identified, the bacteria can be tested for their susceptibility to different antibiotics to determine the most effective treatment. Why it is used To diagnose bone marrow infections: It is used to diagnose infections of the bone marrow caused by slow-growing mycobacteria, which can be difficult to detect. To diagnose disseminated TB: It is particularly useful in diagnosing disseminated or miliary tuberculosis, a serious form of TB where the infection has spread throughout the body, often presenting with nonspecific symptoms. To distinguish between TB and non-TB infections: It helps differentiate between tuberculosis and other bacterial infections in the bone marrow. To monitor treatment: A follow-up culture can be used to check if the treatment is effective. Other related tests AFB smear: This is a quicker microscopic test that can provide a presumptive result in one to two days. However, it must be confirmed with a culture, as a positive smear doesn't confirm a diagnosis of TB. Molecular tests (NAAT): These tests detect the genetic material of mycobacteria and can also provide a presumptive diagnosis, especially when TB is highly suspected.
